By sending this you are asking our team to call you back about an appointment. Please do not include anything you would not want to send by email. Sending this form does not create a doctor and patient relationship, and it is not for emergencies.

Build notes, not patient copy.

The form posts to /api/request-a-call-PLACEHOLDER, a placeholder. The real endpoint is chosen after the HIPAA memo and lands with a signed BAA. On success it redirects to /thank-you/, which is what makes the submission measurable.
